Coastline is currently accepting resumes for a Behavioral Health Clinician in the Home Care Department. The successful candidate will collect medical information that is necessary to evaluate a client’s eligibility for services. The Behavioral Health Clinician will oversee, develop, and implement the guidelines of Intensive Case Management.
On-going collaboration and communication are necessary in working towards the overall health goals of those involved with the consumer including, PCP, Psychiatrist, Neurologist, or Social Worker. Thus, the Behavioral Health Clinician must have a medically-focused background and have the ability to identify problems and obstacles for the well-being plan of the consumer.
Behavioral health services require a team-based approach to care; therefore, we are looking for a great candidate with a variety of keys skills, including synergy, social perceptiveness, time management, critical thinking and excellent communication.
Qualifications

Conducts intensive and therapeutic evaluations to consumers that are experiencing mental health issues that are interrupting them from getting health services through the agency.

Promotes mental and behavioral health with consumers in the community by coordinating necessary services.

Develops and offers educational programming to the agency and to the community on a variety of topics concerning current mental and behavioral health issues for the older adults in our community.

Maintains accurate consumer case records, while re-evaluating & revising service plans on a regular basis to meet new goals.

Ability to multi-task, independent critical thinking, scheduling/balancing field visits.

Bachelor’s Degree in Psychology or a similar field is required.

Master’s Degree in Psychology or a similar field is preferred.

Licensed Clinical Social Worker (LCSW) is preferred.

Experience and familiarity in Mental and Behavioral Health in the community as well as in different environments and settings.

Case Management experience is highly desired.

A minimum of two years’ experience in the Mental and Behavioral Health field as well as a one year experience as a clinician is required.
