Coastline is currently accepting resumes for a Full-Time, Intensive Case Manager in the Home Care Department. The successful candidate will report to the Chief Operating Officer.

The candidate will provide highly focused case management, coordination, advocacy, and support to elders with behavioral health needs who are at risk due to their inability to accept or retain services. The ICM is responsible for operational oversight and professional supervision to the Information and Referral staff. As well as, implementing protocols and procedures assuring compliance with contract regulations while collaborating with all departments and staff within the agency.

Qualifications & Functions

  • Conduct field visits to assess consumer’s physical, social, emotional and environmental status to determine their eligibility and needs in their homes.
  • Collaborate with Care Managers to assist in the development and maintenance of ongoing service plans. Accessing appropriate mental health, medical resources, and legal assistance when cases require such.
  • Develop and implement intensive case management.
  • Manage Information and Referral Department and staff.
  • Maintain a presence on behavioral health task forces and organizing efforts.
  • Perform quality audits on supplemental cases.
  • Experience and familiarity in mental and behavioral health in the community as well as in different environments and settings is required.
  • Home case management and intensive/mental health case management experience is highly desired.
  • Bi-lingual in Portuguese and/or Spanish preferred but not required.
  • Solid communication skills among interoffice staff and other professionals.
  • A minimum of two (2) years’ experience in the mental and behavioral filed is required.
  • Bachelor’s Degree in Social Work or similar field is required.
  • Master’s Degree in Social Work or similar field is preferred.
