Coastline is currently accepting resumes for a full-time Outreach Coordinator in the Elderly Nutrition Program.

The successful candidate will report to the Assistant Director of ENP. The Outreach Coordinator is responsible for the management of home delivered meals to clients and designated meal sites as well as ensuring compliance with EOEA and the agency’s guidelines. The Outreach Coordinator is responsible for maintaining outreach relationships between clients and community programs by optimizing the agency’s nutritional and other support services as needed.

We are looking for a great motivated individual who is team-oriented and ready to work in an integral part of the Elderly Nutrition Program.

The ideal candidate must have exceptional customer service and be able to interact with the elderly in an empathetic, compassionate and sensitive manner.

Functions & Qualifications

  • Conduct initial in home assessment of applicants’ needs and eligibility for services, develop appropriate service plan; make appropriate referrals to home care and services.
  • Conduct complete biannual in home reassessments for Title III clients, review service plan, complete meal service evaluations for home care clients.
  • Advocate for clients; inform and counsel client as to proper nutrition and consult, as necessary, with doctors on dietary needs.
  • Monitor case load via regular follow-up contact and identify clients for changes requiring additional services.
  • Assist with problem solving and make referrals as needed.
  • Assist with applications for public benefits and health insurance and problem resolution related to benefits.
  • Collaborate with other programs and agencies.
  • Communicate to Nutritionist and Food Service and Congregate Program Supervisor regarding vendor quality issues.
  • Utilize SAMS to manage caseload, update client records, document phone calls, visits, and all activity to maintain client records accurately and within the mandated timeframe.
  • Utilize SAMS reports to assure compliance with guidelines and prepare accurate delivery route sheets.
  • Interview and assess needs of site participants.
  • Inform and counsel participants about nutritional needs individually, as well as in group.
  • Maintain and update participants’ files.
  • Perform outreach activities for the purpose of identifying, recruiting, and involving target population in Program activities.
  • Implement recreational and social services for participants with the assistance of the Food Service and Congregate Program Supervisor.
  • Provide office and nutrition site management coverage and deliver home delivered meals, as needed.
  • Prepare caterers’ orders.
  • Break down and assemble site pouches.
  • Assist Program Director and Nutritionist in strengthening programs in the community.
  • Help coordinate programs such as holiday meals, Homebound and Farmers’ Market Coupon Program, and other activities throughout nutrition sites.
  • Attend courses, workshops, and In-service training for professional development.
  • Meet with Supervisor and other staff at regularly scheduled intervals.
  • Plan and coordinate services and programs which support project goals.
  • May be required to perform other duties as required.
