Coastline is currently accepting resumes for a full-time (35 hours per week) Options Counselor & Family Caregiver Specialist within our Community Resources Program at an hourly rate from $24–$27.

The successful candidate will report to the Community Resources Supervisor. The Options Counselor/Family Caregiver (OC/FC) Specialist is a trusted guide who is responsible for providing information related to community-based long-term care services and supports with the goal of improving quality of life, including for individuals living with dementia. This role also includes responding to inquiries from the community regarding the Options Counseling and Family Caregiver Programs.

The ideal candidate possesses deep knowledge of care options and community resources, excellent communication and problem-solving skills, and the ability to build trust with families while empowering them to access the supports they need.

A Bachelor’s Degree in Social Work, Gerontology, Psychology, Sociology, or a related field is required with a minimum of three (3) years of experience in human services environment, including at least one (1) year of experience working with one of the target populations. The ideal candidate must be reliable, organized, and bring strong caregiving skills that support and advance Coastline’s mission and vision.

Pay Rate: $24–$27

Essential Functions

  • Provide counseling regarding community-based services and supports at the setting requested by the participants.
  • Contact consumers to discuss and provide guidance regarding benefits and supports while considering consumers’ preferences.

  • Assist consumers in developing plans that are consumer-directed and include services, programs, financial resources and other supports available to consumers.

  • Provide comprehensive services and supports for people with dementia and their caregivers. Guiding an Improved Dementia Experience (GUIDE).

  • Deliver clear, well-written informational materials and presentations to community based organizations, healthcare facilities, health fairs and education forums.

  • Assist with referrals and maintain accurate documentation for contacts, progress notes, surveys, and quality outcome reporting.

  •  Participate in team and agency staff meetings as required.

  • Promote and explain the agency’s programs and services to the community through outreach activities.

  • Maintain caregiver resources, identify and address unmet needs, provide education and support, and perform caregiver assessments to connect families with appropriate resources.

  • Perform other duties as required.
