Nominate a Caregiver
NOVEMBER IS NATIONAL CAREGIVERS’ MONTH
Coastline is inviting you to nominate a family member, friend, or associate who’s gone above and beyond to care for someone in our community. Your nominations will help us shine a light on the selfless dedication and hard work shown by caregivers every day.
Caregivers must meet the following criteria:
- Must be currently caregiving for someone in the community in Coastline’s service area;
- Can be any age caring for an older adult or person with a disability;
- Can be a grandparent any age caring for a grandchild; and,
- Should be someone who has positively impacted their care receiver’s life.
- A photo (JPEG or PNG version) must be provided of caregiver. (Photos must be emailed to jkopeckylanausse@coastlinenb.org. Please put caregiver’s name in subject line.)
Coastline will highlight select caregivers during the month of November through our social media channels. Three nominees will be chosen to receive special recognition and a “Caring for the Caregiver” gift basket.
To make a nomination, complete and submit this form and email a photo of the nominee with their full name to Jessica at jkopeckylanausse@coastlinenb.org.
Nominations can also be made over the phone by calling 508-999-6400 and asking to speak with a family caregiver support specialist. Nominations must be received by 5 p.m. on Oct. 31.
