A dementia diagnosis for yourself or someone you love can feel overwhelming. You may have questions about what comes next, how to get the right care, or how to take care of yourself while caring for someone else.
Coastline now offers a new program that can help: GUIDE, which stands for Guiding an Improved Dementia Experience is a federal program through Medicare designed to bring more support, more coordination, and more breathing room to families facing dementia.
What is GUIDE?
GUIDE is a Medicare program for people living with mild, moderate, or severe dementia, and for the caregivers who support them. It offers a team of professionals who work with your family to build a care plan that fits your specific needs and adjusts as those needs change over time.
The goal is simple: better quality of life, less stress, and a caregiver who isn’t carrying the weight alone.
There is no cost to you for these services.
What support does GUIDE offer?
- A personalized care plan built around your situation, and updated as things change
- A full assessment to understand what’s working, what’s hard, and what kind of help would make the biggest difference
- Regular check-ins so you always have someone to turn to with questions
- Caregiver training and education, so you feel more confident and prepared
- Respite care to pay for in-home help or adult day center care, giving caregivers real time to rest
Do you qualify?
You may be eligible if:
- You (or your loved one) have been diagnosed with mild, moderate, or severe dementia
- You have Original Medicare, Parts A & B (not a Medicare Advantage plan)
- You are not currently enrolled in a Senior Care Option (SCO)/One Care, PACE, or hospice
