Coastline is currently accepting resumes for a full-time Geriatric Support Services Coordinator in our Senior Care Options Program at an hourly rate of $27.50.

The successful candidate will report to the Senior Care Options Supervisor. In this role, the Geriatric Support Services Coordinator will provide dynamic leadership by assessing clients’ needs and developing, implementing, and monitoring individualized service plans through a variety of communication methods. We are seeking a compassionate and dedicated professional with a strong understanding of social work principles and practices, who is committed to providing exceptional support to the aging population.

A Bachelor’s Degree in Social Services is desired and two years professional experience with the aging population over 60, with at least one year in a setting where the aging receive health care services is desired.

The ideal candidate will be dependable, organized, and possess excellent assessment, communication, and care coordination skills that support and advance Coastline’s mission and vision.

Pay Rate: $27.50

Qualifications

  • Ability to identify and critically assess consumers’ needs while maintaining accurate documentation and ensuring compliance with record management requirements.
  • Excellent written and basic computer skills.
  • Ability to communicate effectively within a fast-paced team environment.
  • Fluency in Spanish or Portuguese is desired, but not required.
  • Knowledge of aging services, community resources, and support programs.
  • Self-motivated or driven to perform consistent outstanding work.
  • Reliable transportation and a valid Massachusetts driver’s license are required.
